SCHOOL PHYSICALS
PLEASE SCHEDULE EARLY
required for;
- Participation in sports (required yearly)
- Kindergarten
- 5th grade
- 9th grade

Please bring the following:-
- The form the school provided
- Your child’s immunization record
- A urine specimen (keep refrigerated) or have the child prepared to give one in the office.
Children are welcome to wear their swim suit or swimming trunks under their clothing.
School physicals are NOT covered by all insurance companies. Please call your insurance to inquire about benefit coverage. |
